Very concerning that you feel qualified to treat me and have access my scans & history etc, but unable to complete a simple form.
I guess no money in a patient who cannot any longer be repaired hey Allan? Best get your facts straight when your telling tales, you never know which of your clients have done their research. Shame on you..
