Client / Responsible Party (RP): Confirms needs, participates in decisions, agrees to services, signs/acknowledges the plan (when required).
• Assessing RN / Case Manager (payer or state program): Performs assessment, determines needs, and develops/approves the service plan under the program rules (often the “official” plan you must follow).
• Primary Care Provider (PCP) or Ordering Provider (when required): Certifies medical necessity or signs orders if the program requires it.
• A&M (Agency Administrator/Clinical Supervisor): Ensures the POC is obtained, accurate, staffed, followed, and updated; trains staff and monitors compliance.
