Data collected by community health needs assessment surveys conducted by Community Health Network, St. Vincent Health, the Franciscan St. Francis Health and IU Health, identified three health priorities for Hamilton County: access to healthcare, improving health behaviors and increasing availability to mental health services and expanding preventive health screenings and education. Using this data as a guide, PHHC’s primary focus will be to address these needs in order to make a positive impact on the health of Hamilton County residents.
A community health needs assessment is a report completed by hospital organizations, public health agencies and other stakeholders to improve community health by identifying priority health needs and adopting implementation improvement strategies to address them. The report is based on data collected through surveys, focus groups and town hall meetings, as well as direct engagement of community residents in research efforts. PHHC priority areas are based on this data. They are access to healthcare, health behaviors and mental health and preventive health and screenings.
