Then there are a few Care Coordinators that stand out. Those that are master collaborators – building teams of service providers, family members, friends, and people in the community which form a tight knit circle around a person who needs supports. If you’ve got one of these people in your corner, thank him or her. If you don’t, I’d welcome the opportunity to sit and visit with you for a while.
