    Today can get the Medical certificate form here.
    No rush at all, only two were at the moment including me.
    We already completed our medical exam
    You can come directly or give call and take the time.

    Place and staff well organized., But this place is so bad that allocated by the Government. They should facilitate for parking if they establish this type of area. We do not need parking if very closer to city. Not maintaining nice environment. We need to remove shoes on the floor that made with big rock and මඩ. That is not acceptable Place for the government institution .

    These are the best days to have your medical certificate.dial 225 and made the appointment.be there 30 minutes earlier.well organized service given by them thanks for corona.highly appreciated service and staff.i got my medical certificate within one and half hour.

    It's operating on appointment basis only , call 225(e-channeling) and place an appointment for Driving License medical test. Charges around Rs900, cost will be deducted from your mobile account balance.
    Bring NIC(or Passport) and Driving License(if owned) and a pen. Wear a loose trouser as you need to fold it up above the knee level. Whole process takes about 2 hours.
    Enough parking options are available around the area for a small cost. Supportive staff (specially front officers) and efficient service.

    This place is already booked at 5.30 am by some other driving school people.
    People who are standing has to wait even under the rain.
    Very bad and an unpleasant situation to experience.
    No online bookings available.
    Seems Mr presidents (GR's) plans not welcomed by Gampaha office 😅

    I went to NTMI around 6:00am & waiting queue. Around 6:30 am they start work there. I got token 135. Entrance method little bit away from organized. But inside service well organized, highly appreciate. Finaly i got my medical around 10 am.

    There is no parking space, but who are the people living around this office provided parking with small charges.

    Today I went to NTMI - Gampaha around 5.15 a.m and my token number was 93 . They open office by 6.45 a.m for the service. Inside operation highly organised and fast. I got my medical certificate by 9.20 a.m .

    NTMI should open more branches to reduce queue .

    You have to come here to get the medical report for the driving license. People are come here around 4AM to get in front of the queue. They are giving numbers to obtain the service. No parking space available here, but people who lives around here let you park your vehicles in their home garden for 100 rupees.

    The entrance method is a bit way out of organized, although i was able to go inside relatively fast. The security calling out the numbers cannot be heard at the back. Some of the members are hostile although the rest is very friendly and helpful. The system could be improved by introducing some more technology to the existing.

    The place is very small and its not well organized. People have to wait hours to get their work done. I got there at 8 a.m. and the number I got was 322. I had to wait till 12.30p.m to get the report. Staff is trying their best but some officers act rudely.

    Small lobby area but well organized staff highly efficient if you go there before 5.00am you can get your medical report before 10.00am in advance wear a loos trousers or short, two food corners available here, tuk tuk available after 9.00am to go to kachcheriya (Rs 120/= only) and private parking also available here

    Not properly organized. People from 2am waiting to get numbers. Very small place. Good thing is everyone treated equally.
    As of noe new system and facilities to be improved. Calling numbers can not hear back of the crowed. At least to place large screen to visible which number is proceed.
    There are many things to improve. Had bad experience

    IMPORTANT!!! if you are coming here on a week day, make sure to get here early, 4 - 5Am is recommended, and if your arriving here on the train then know that you may not find tuk tuk's near station during that time, if you somehow arrived during 4 a.m. to 5:00 a.m. then you will likely be done by 9:00 a.m. or 10 a.m. Make sure you'll bring an umbrella if the weather is bad because you don't have much to find shelter in, the service was actually better than I expected but it can most definitely get better.

    I went there to get medical certificate at 8.00 am today.Further I get my medical certificate @ 11.00 am.My number was 256.Good staff is working.Efficencicy is good.But lack of facillities.But you have chanagable money notes.Other wise you have wait to change money.

    As at 21 st December 2018, the procedure of obtaining a medical certificate to obtain a new/renew your driving licence is as follows. You need to take your Original Birth Certificate, NIC and Old Driving License if available. If you are coming on a vehicle other than a tuk or a bike you will have to park your vehicle at nearby Shri Bodhi Road. From 6am onwards forms are issued from counter 1 which is situated at the very begining of the building. Your number is indiacated in this form. You need to fill this form and wait at the outside. Then they start to take the applicants inside according to the order of the number you got. For example people from 1 to 20 are called first. Then 21 to 40.. When your number is called, you need to go to counter number 2. From their your weight and height is measured and your photo is taken. From counter number 2 onwards you need to go to counters 3,4,5,6 and 7 according to the order and way to each counter is clearly indicated on the walls. You need to pay to the cashier in counter 3 and obtain the receipt. From counter 4, a blood check is carried out. Then at the counter 5 you need to sign in your medical certificate confirming that you have no prior certain illnesses. Then at counter 6, a doctor will examine you. There your eye sight, hearing and bending of arms, legs, fingers and toes will be throughly checked. If you have any previously obtained X-rays you need to show that to this doctor. After that you need to stay at the waiting area and when your name is called you can get your medical certificate from counter 7.

    The process is not that hard to follow. Everything is clearly mentioned on the walls as notices. The place has limited place and staff is trying thier best to give a good service. But the behaviour of some of the staff (1 or 2) is not good. Do not know how to talk with elderly people. These few people ruin what others are trying to achieve with limited facilities.

    You should go there around 5am but after 7am the rush reducing. Keep remember maximum number of checkup limited to 500. Staff are little bit rude.

    7 sections, complete all to get the damn medical.
    red seats are where you can read your books till 12 and wait till they call your number. 4 exit points in case of an invasion.. run outside and towards the road .. there will be a gas station ..blow it up.. the look on their face will be epic.
    #victoryroyale was it worth it tho? No, I would order an attack on that place

    The place was horrible. The staff was very rude, not just to me, but everyone who was there. Service was not so efficient, specially at the last counter. I would like to highlight ths point about how the staff treates the people there. Maybe there job is stressful, but there are many other people who have harder jobs and come from very far away to get there medical report. They didn't even provide proper instructions and started scolding the people there for not following instructions, which they didn't even mention the first place. This place is horrible and I wouldn't go back, even if my license has expired. Once again, the staff was extremely rude and the way they treated the people there was very disgusting.

    Staff is working with in full capacity. an efficient service by minimum staff. Giving token starts around 6:15 AM. I joined the queue at 6:05 and got number 86. Your token number is on the form given by the staff. Every step is organized in to a organized manner. All the instructions are displayed. You must bring your NIC and Driving license only for renewal. They take a photo for medical certificate. No need to bring birth certificate or any other or photocopy of NIC and driving license. You must bring a pen. Have to remove shoes before entering the office (counter one). So if you can bring a bag to hold shoes, it is easy for you. Don't wear socks and shoes. Try to come wearing some dandles. counter 2 is registration, 3 is payment, 4 is blood sample taking. after 4, you have to fold your trouser up to knee to clearly show the knees. so it is easy to wear a short by gents and skirt by ladies. Motor bikers must park their bikes inside the office premises (please do not park on entrance road) and no space for parking of cars in office premises. Cars can be parked on the road adjacent to petrol shed (Sri Bodhi Rd). always follow the instructions given by the staff. They are very helpful and kind.

    Efficient Service. I’ve joined the que by 6am collected the medical report by 8.58am.

    The medical institute starts at 7am. People starts the que from 6am.

    Once you enter the institution takes as 50 batches and send you through 7 counters

    Counter1: Weight & Height measure
    Counter2: Registration
    Counter3: Cashier
    Counter4: Blood Group, Blood Glucose level test, Blood pressure, lung test, Checking for physical disability & Vision check
    Counter5: Declaration
    Counter6: Waiting
    Counter7: Issuance of Medical Reports

    Notes.

    All medical testing fees 800LKR.

    At counter 4 only the males need to show the foot up to knee length. This needs to fold your trousers above knee length. Better to where loose trousers.
